Job summary

Amedisys is seeking a Registered Nurse to provide high-quality home health care with an emphasis on whole-patient well-being. You will perform patient assessments, collaborate with the care team, and implement individualized plans of care in a supportive orientation during your first 90 days.

Qualifications

• 1+ year of clinical RN experience.
• Current RN license for the assigned state(s).
• Current CPR certification.
• Valid driver’s license with reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs and documents rationale.
• Promotes patient health and independence through teaching and rehabilitative measures.
