Job summary

Amedisys is hiring a registered nurse case manager in a full-time role with flexible day schedules in the United States. You will assess patients, develop care plans with the team, and supervise LPNs and HHAs while ensuring timely documentation.

Qualifications

• One (1+) year of clinical RN experience required.
• Current RN license valid for assigned state(s).
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs and documents rationale.
• Promotes health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs; completes timely and accurate documentation.
• Communicates patient progress to clinical manager and care team; plans staff education.
• Performs on-call responsibilities and participates in continuing education.
