Job summary

Amedisys Home Health seeks a Registered Nurse to coordinate patient care in home settings, performing assessments, treatments, education and care planning. You will work with a clinical team to tailor plans and ensure high-quality, patient-centered care in the home environment.

Qualifications

• One year of RN clinical experience.
• Current RN license for the state(s) assigned.
• Minimum six months home health experience.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.
• Ability to assess patient status and needs by age-specific requirements.
• Familiarity with basic computer use and online HR/training systems.
• Note: If <6 months RN experience, RN Intern program may be required.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Gives injections, IV therapy or tube feedings.
• Administers and maintains catheters.
• Completes documentation timely and accurately.
• Supervises LPNs and HHAs.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Other duties as assigned.
