Job summary

Amedisys is seeking a Registered Nurse Case Manager to join our home health team. This role focuses on patient assessments, care planning, and collaboration with the care team to support healing at home.

Qualifications

• 1+ year of clinical RN experience required.
• Current RN license valid for state(s) assigned.
• CPR certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Participates in on-call responsibilities and services.
• Engages in clinical development and continuing education.
