Job summary

Alc-Health-Care-Services is seeking experienced home health care RNs for per diem assignments in Illinois. The role involves evaluating patient needs, initiating and updating care plans, and delivering nursing treatments in patients' homes and facilities.

You will administer medications, counsel patients and families, and coordinate with physicians and caregivers. A valid IL RN license and CPR certification are required, along with a valid driver’s license and insured vehicle.

Qualifications

• Minimum of 2 years of home health care experience.
• Current RN IL license and CPR certification.
• Licensed driver with insured vehicle and good working order.
• Excellent observation, verbal and written communication, problem solving, and basic math skills.

Responsibilities

• Regularly re-evaluates patient nursing needs.
• Initiates and revises the plan of care as patient status changes.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals and family involvement.
• Administers medications and treatments as prescribed.
• Counsels patient and family to meet nursing needs.
• Provides health care instructions and discharge planning as appropriate.
