Job summary

ALC Health Care Services is seeking experienced home health care RNs for per diem, on-the-road assignments across Illinois. You will evaluate patients, initiate and adjust care plans, and provide nursing care and education to patients and families.

Responsibilities include administering medications, coordinating with physicians, and coordinating equipment and services while maintaining high-quality documentation and communication.

Qualifications

• Minimum of 2 years of home health care experience.
• Current RN IL license and CPR certification.
• Good communication and nursing skills.
• Ability to lift and transfer patients safely.

Responsibilities

• Regularly re-evaluates patient nursing needs.
• Initiates plan of care and revises as patient status changes.
• Uses assessments to determine nursing diagnosis.
• Develops care plans with patient/family involvement.
• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed.
• Counsels patient and family on nursing needs.
• Provides care instructions per assessment and plan.
