Job summary

ALC Health Care Services is seeking experienced home health care RNs to provide per diem visits in patients’ homes and facilities across Illinois. You will assess, plan, implement, and document nursing care with patient and family involvement.

The role requires strong clinical skills, CPR certification, and a valid IL RN license. Mileage reimbursements and flexible scheduling are available to support travel and workload demands.

Qualifications

• Two years of home health care experience.
• Current RN IL license and CPR certification.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Re-evaluates patient nursing needs regularly.
• Initiates and revises the plan of care as patient status changes.
• Uses health data to determine nursing diagnosis.
• Develops care plans with patient and family involvement.
