Job summary

ALC Health Care Services seeks an experienced home health care RN to perform per diem nursing visits across facilities and patients at their homes in Morris, IL. The role involves conducting assessments, implementing plans of care, and coordinating with physicians and families.

Responsibilities include re-evaluating patient needs, administering medications, educating patients, and arranging discharge planning as needed. Valid IL RN license and CPR required; driving is essential.

Qualifications

• Minimum of 2 years of home health care experience.
• Current RN IL license and CPR certification.
• Good observation, verbal and written communication skills; basic math; nursing skills per competency checklist.

Responsibilities

• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and revises as patient status changes.
• Uses health data to determine nursing diagnosis.
• Develops a care plan with goals and family involvement.
• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed.
• Counsels patient and family with meeting nursing needs.
• Provides care instructions per assessment and plan.
• Identifies discharge planning needs and prepares prior to discharge.
• Prepares clinical notes and updates the physician as needed.
• Coordinates with community health-related persons; participates in on-call duties.
• Ensures arrangements for equipment and services are available.
