Job summary

Alc-Health-Care-Services is seeking experienced home health care RNs to provide nursing care in facilities and patients' homes. Responsibilities include re-evaluating patient needs, initiating and revising care plans, administering medications, and counseling patients and families.

Per diem position with flexible scheduling and mileage reimbursement. Minimum 2 years home health care experience required, with a current RN IL license and CPR certification.

Qualifications

• Minimum of 2 years of home health care experience.
• Current RN IL license and CPR certification.
• Licensed driver with an automobile in good working order.

Responsibilities

• Regularly re-evaluates patient nursing needs and revises plans as needed.
• Initiates and updates the care plan with family involvement.
• Administers medications and treatments as prescribed.
• Counsels patient and family on nursing and related needs.
• Provides care instructions and identifies discharge planning needs.
• Prepares clinical notes and updates the primary physician as required.
• Communicates patient status and coordinates with community health professionals.
• Participates in on-call duties and arranges equipment/services as needed.
