Job summary

ALC Health Care Services seeks experienced home health care RNs to provide nursing care in patients’ homes and facilities. The role involves developing and updating care plans, administering medications, and coordinating with physicians and families.

Applicants must hold a current IL RN license and CPR certification, have at least 2 years of home health experience, and be a licensed driver with a suitable vehicle. Per diem schedule offers flexibility and mileage reimbursement.

Qualifications

• Must have current IL RN license and CPR certification.
• Minimum 2 years of home health care experience.
• Licensed driver with insured vehicle as required by state.
• Ability to lift and transfer patients and perform related tasks.

Responsibilities

• Regularly re-evaluates patient nursing needs.
• Initiates and revises the plan of care as needed.
• Uses health data to determine nursing diagnosis.
• Develops care plans with patient and family involvement.
• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed.
• Counsels patient and family on nursing needs.
• Provides health care instructions per assessment and plan of care.
• Identifies discharge planning needs and prepares accordingly.
• Communicates with physicians and coordinates care plan.
