Job summary

VNA of Florida is seeking a Registered Nurse (RN) for our Home Health program serving Lake County including Leesburg, Eustis, Mount Dora and Clermont. The RN will coordinate and deliver high-quality home-based nursing care, manage cases, and collaborate with physicians and the interdisciplinary team.

This full-time role requires Florida RN licensure, at least one year of nursing experience, and ideally home health and OASIS familiarity.

Qualifications

• Active Florida RN license.
• Minimum of 1 year nursing experience.
• Home health experience preferred.
• OASIS experience preferred.
• Strong assessment and critical thinking skills.
• Ability to work independently.

Responsibilities

• Plans, coordinates, and ensures quality home healthcare services for patients and families.
• Delivers professional nursing services in the home and provides case management for primary patients.
• Monitors client needs and adjusts plans of care.
• Performs initial assessment and tracks progress toward goals.
• Maintains communication with patient Families physicians and staff.
