Job summary

The Lakes Home Care, Inc. is seeking a dedicated Registered Nurse to join our Home Health team, applying the nursing process to deliver patient care in a home setting. You will assess, plan, implement and evaluate care under the direction of the Director of Clinical Services.

Qualifications

• Current RN license in the state of practice.
• Current health certificate per contract or state regulation.
• Current PPD or Chest X-Ray.
• Current BLS certification.
• One year RN experience preferred.

Responsibilities

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data from health care members and documents nursing diagnoses.
• Document the patient’s plan of care with diagnoses, outcomes and interventions.
• Performs interventions according to priorities, plan of care and hospital policies.
• Revises the plan of care as evaluation shows changes in medical plan or interventions.
• Use clinical judgment in evaluation activities to meet patient needs on assigned unit.
• Other Registered Nurse duties as assigned.
