Job summary

The Lakes Home Care, Inc. is seeking a dedicated Registered Nurse to join our Home Health team, applying the nursing process to deliver compassionate patient care under the Director of Clinical Services.

You will work within a home-care setting to assess, plan, implement, and evaluate patient care in collaboration with the care team. Requirements include an active RN license, health certificate, BLS, and at least one year of RN experience.

Qualifications

• Current RN license for practicing state.
• Current Health Certificate per facility or state regulation.
• Current BLS card.

Responsibilities

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes.
• Document the patient’s plan of care using nursing diagnoses, outcomes, and interventions.
• Perform interventions according to priorities and plan of care.
