Job summary

The Lakes Home Care, Inc. is seeking a dedicated Registered Nurse to join our Home Health team in Florida. You will apply the nursing process, assess and plan patient care, and deliver skilled nursing services under the direction of the Director of Clinical Services.

You will work with a 1:1 patient ratio, enjoy flexible scheduling, and contribute to compassionate community-based care for medically fragile patients in your area.

Qualifications

• Current RN license in the state of practice.
• Current health certificate.
• Current PPD or Chest X-ray.
• Current BLS certification.
• One year RN experience preferred.

Responsibilities

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collect, analyze, and interpret data from health care members and document nursing diagnoses.
• Document the patient’s plan of care with expected outcomes and interventions.
• Perform interventions according to priorities, plan of care, and hospital policies.
• Revise the plan of care according to evaluation and changes in medical plan.
• Use clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor.
• Other Registered Nurse (RN) duties as assigned.
