Job summary

The Lakes Home Care, Inc. is seeking a dedicated Registered Nurse to join our Home Health team in Florida. The RN will apply the nursing process and deliver patient care under the direction of the Director of Clinical Services.

Responsibilities include comprehensive nursing assessment, data collection, documentation, and revision of care plans. Flexible hours and competitive salary are offered to support quality home care for medically fragile patients in the community.

Qualifications

• Current RN license for the state of practice (Florida).
• BLS certification is required or in progress.
• Health certificate and PPD/chest x-ray as per facility policy.

Responsibilities

• Apply the nursing process to assess, plan, implement, and evaluate patient care.
• Monitor signs of physiologic and psychosocial changes in patients.
• Document plan of care with diagnoses, outcomes, and interventions.
• Follow orders and revise care plan based on evaluation and changes.
