Job summary

The Lakes Home Care, Inc. is looking for a dedicated Registered Nurse to join our Home Health team.

The RN will apply the nursing process and deliver patient care under the direction of the Director of Clinical Services, exercising clinical judgment to ensure quality outcomes in the home setting. Responsibilities include assessment, planning, implementation, evaluation, and documentation of the plan of care.

Qualifications

• Current RN license for the state of practice.
• Current Health Certificate or equivalent per facility.
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year RN experience preferred.

Responsibilities

• Utilize the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient.
• Document the plan of care with identified nursing diagnoses and expected outcomes.
• Perform interventions according to priorities and approved plan of care.
• Revise the plan of care as evaluation and medical plans evolve.
• Apply clinical judgment to meet patient care needs of the assigned unit or home setting.
• Other RN duties as assigned.
