Job summary

Visiting Nurse Association of Florida is seeking a Registered Nurse (RN) for home health in the Stuart/Hobe Sound area. This PRN role offers flexible scheduling and the opportunity to deliver high-quality in-home care through a trusted nonprofit with 50 years of service.

The RN will perform skilled visits, complete OASIS assessments, and develop individualized care plans while educating patients and caregivers.

Qualifications

• Active Florida RN license.
• Minimum one year of nursing experience.
• Home health experience preferred.
• OASIS experience preferred.
• Strong assessment and critical thinking skills.
• Ability to work independently.

Responsibilities

• Perform skilled nursing visits in patient homes
• Complete OASIS assessments and documentation
• Develop and manage individualized care plans
• Educate patients and caregivers on disease management and safety
• Coordinate care with physicians and interdisciplinary team members
• Monitor patient progress and communicate changes in condition
