Job summary
BrightStar Care is seeking a compassionate Home Health Registered Nurse (RN) to provide high-quality, in-home patient care across the service area in Texas. You will perform SOC, Recertifications, care planning, wound care, lab draws, and education for patients and families, coordinating with physicians and the care team to optimize outcomes.
Ideal candidates enjoy autonomous work, strong assessment skills, effective communication, and reliable transportation for frequent home visits.
Qualifications
- Current Texas RN license in good standing.
- CPR/BLS certification required.
- Minimum one year of nursing experience.
- Valid driver's license and reliable transportation.
- Strong assessment, organization, and communication skills.
Responsibilities
- Complete SOC, Recertification, Resumption of Care, Evaluation, and Discharge visits.
- Perform comprehensive nursing assessments and develop individualized care plans.
- Monitor patient progress and communicate changes to physician and care team.
- Coordinate home health services with physicians, therapists, patients, and caregivers.
- Provide wound care, collect lab specimens, and administer medications as ordered.
- Educate patients and caregivers on disease processes, medications, wound care, safety, and self-management.
