Job summary

BrightStar Care is seeking a compassionate Home Health Registered Nurse (RN) to provide high-quality in-home care. You will perform Start of Care visits, assessments, care planning, and education for patients and families in their homes.

The role requires a Texas RN license in good standing, CPR/BLS certification, and at least one year of nursing experience. A valid driver’s license and reliable transportation are required to reach patient homes across the service area.

Qualifications

• Texas RN license in good standing.
• Current CPR/BLS certification.
• Minimum one year of nursing experience.
• Strong assessment, organization, and communication skills.
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsibilities

• Complete Start of Care (SOC), Recertification, Resumption of Care, Evaluation, and Discharge visits.
• Perform comprehensive nursing assessments and develop individualized plans of care.
• Monitor patient progress and communicate changes in condition to the physician and care team.
• Identify patient needs and coordinate appropriate home health services.
• Coordinate care with physicians, therapists, patients, caregivers, and community resources.
• Collaborate with interdisciplinary team members to ensure continuity of care.
• Participate in case conferences and care plan updates.
• Assess patients for home health services eligibility.
• Identify needs for PT, OT, and ST and coordinate evaluations.
• Coordinate therapy evaluations and ongoing services as appropriate.
• Perform routine and specialized nursing procedures.
• Provide wound care and wound assessments.
• Collect laboratory specimens and perform lab draws as ordered.
• Administer medications and monitor treatment effectiveness.
• Manage chronic disease conditions and post-hospital recovery care.
• Teach and train patients and caregivers on disease processes, medications, wound care, safety, infection control, self-management, and device use.
• Complete accurate and timely electronic documentation.
• Ensure compliance with home health regulations and HIPAA.
