Job summary
BrightStar Care is seeking a compassionate Home Health Registered Nurse (RN) to provide high-quality care to patients in their homes across the Texas service area. The RN will perform Start of Care visits, ongoing assessments, wound care, lab draws, and care coordination with physicians and the interdisciplinary team.
The ideal candidate holds a Texas RN license, CPR/BLS, a minimum of one year of nursing experience, and a valid driver’s license.
Qualifications
- Current Texas RN license in good standing.
- Current CPR/BLS certification.
- Minimum one year of nursing experience.
- Strong assessment, organization, and communication skills.
- Valid driver's license, reliable transportation, and current auto insurance.
Responsibilities
- Complete SOC and other regulatory visits and evaluations.
- Perform comprehensive nursing assessments and develop individualized plans of care.
- Monitor patient progress and communicate changes to the physician/care team.
- Coordinate home health services with physicians and other care providers.
- Educate patients and families on disease processes, medications, wound care, safety, and self-management.
