Job summary

BrightStar Care is seeking a compassionate Home Health Registered Nurse (RN) to provide high-quality, in-home care to patients across Texas. You will perform SOC visits, coordinate care, educate patients and families, manage wound care, and collect lab specimens, collaborating with physicians and the care team to optimize outcomes.

This role offers autonomy and meaningful relationships with patients and families, delivering excellent care in home settings, with a supportive clinical team.

Qualifications

• Current Texas Registered Nurse (RN) license in good standing.
• Current CPR/BLS certification.
• Minimum one year of nursing experience.
• Strong assessment, organization, and communication skills.
• Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Perform Start of Care (SOC), Recertification, Resumption of Care, Evaluation, and Discharge visits.
• Coordinate care with physicians, therapists, patients, caregivers, and community resources.
• Assess patients for home health services eligibility and coordinate therapy evaluations as needed.
• Provide routine and specialized nursing procedures, wound care, and lab draws.
• Educate patients and caregivers on disease processes, medications, wound care, safety, and self-management procedures.
• Complete accurate electronic documentation and ensure HIPAA compliance.
