Job summary

BrightStar Care is seeking a compassionate Home Health Registered Nurse (RN) to provide high-quality care in patients' homes across Texas. The role covers SOC, care planning, wound care, lab draws, and medication administration with interdisciplinary collaboration.

The ideal candidate will have Texas RN licensure, CPR/BLS, at least one year of nursing experience, and strong assessment and communication skills to support patient independence and recovery.

Qualifications

• Current Texas RN license in good standing.
• Current CPR/BLS certification.
• Minimum one year of nursing experience.
• Strong assessment, organization, and communication skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete SOC, Recertification, Resumption of Care, Evaluation, and Discharge visits.
• Perform comprehensive nursing assessments and develop individualized plans of care.
• Monitor patient progress and communicate changes in condition to the physician and care team.
• Coordinate care with physicians, therapists, patients, caregivers, and community resources.
• Assess patients for home health services eligibility and coordinate therapy evaluations.
• Provide wound care, lab draws, medication administration, and chronic-disease management.
• Educate patients and caregivers on disease processes, medications, safety, and self-management.
• Maintain accurate electronic documentation and HIPAA-compliant practices.
