Job summary

BrightStar Care seeks a compassionate Home Health Registered Nurse (RN) to provide high-quality care in patients' homes across the service area in Texas. You will perform SOC/start-of-care visits, assessments, care plans, and collaborate with physicians and the interdisciplinary team to improve outcomes.

Responsibilities include wound care, lab draws, medication administration, education for patients and families, and thorough documentation while maintaining HIPAA compliance.

Qualifications

• Current Texas RN license in good standing.
• Current CPR/BLS certification.
• Minimum one year of nursing experience.
• Valid driver's license and reliable transportation.
• Strong assessment, organization, and communication skills.

Responsibilities

• Complete Start of Care (SOC), Recertification, Resumption of Care, Evaluation, and Discharge visits.
• Perform comprehensive nursing assessments and develop individualized plans of care.
• Monitor patient progress and communicate changes in condition to the physician and care team.
• Identify patient needs and coordinate appropriate home health services.
• Coordinate care with physicians, therapists, patients, caregivers, and community resources.
• Collaborate with interdisciplinary team members to ensure continuity of care.
• Participate in case conferences and care plan updates.
• Assess patients for home health services eligibility including PT/OT/ST as needed.
• Coordinate therapy evaluations and ongoing services as appropriate.
• Perform routine and specialized nursing procedures including wound care and lab draws.
• Administer medications and monitor treatment effectiveness.
• Manage chronic disease conditions and post-hospital recovery care.
• Teach patients and caregivers about disease processes, medications, wound care, safety, and self-management.
• Complete accurate and timely electronic documentation and ensure HIPAA compliance.
