Qualifications

• 2+ years' nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Valid driver's license and own vehicle.

Responsibilities

• Assess and evaluate patient’s status; write and initiate plan of care.
• Re-evaluate patient and family needs; revise plan of care as needed.
• Develop and implement care plans with patient/family input.
• Administer medications and treatments as prescribed by physician.
• Document clinical notes and report changes to physician.
• Ensure safe environment and attend interdisciplinary meetings.
• Participate in on-call duties and care coordination.
