The organization ranked among Boston Globe and USA Today Top Places to Work and offers growth opportunities. The role is a full-time, Monday–Friday position serving Cambridge and surrounding areas, with a salary range of $75,000–$100,000.

Qualifications

• 2+ years nursing experience, preferably in home health or geriatrics.
• RN degree from an accredited program.
• Proficient with electronic medical records (EMR).
• Excellent bedside manner and communication skills.
• Working knowledge of healthcare laws/regulations.
• Maintain current BLS CPR certification.
• Valid driver's license and reliable vehicle.

Responsibilities

• Assess and evaluate patient status and update plans of care.
• Write and initiate plans of care with patient and family involvement.
• Re-evaluate patient and family needs regularly and adjust plans.
• Develop care plans with goals based on nursing diagnosis.
• Administer medications and treatments as prescribed.
• Document clinical notes and changes in patient condition.
• Communicate with physicians about patient needs and orders.
• Maintain a safe home care environment and attend meetings.

Responsibilities:

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.

Qualifications:

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
