Job summary

Well Care Health seeks a compassionate Home Health Registered Nurse to assess and monitor patients in their home environments, develop and implement individualized care plans, and educate patients and families on health management.

The role requires NC/SC nursing licensure, CPR certification, and about 1 year of RN experience with home health preferred. Strong communication, caseload management, and flexible scheduling are essential.

Qualifications

• Graduate of an accredited or approved school of nursing (AD, Diploma, or BSN).
• Current license to practice professional nursing in NC/SC.
• One year RN experience; home health preferred.
• CPR certification required.

Responsibilities

• Assess and monitor patients in their home environment.
• Develop and implement individualized care plans.
• Educate patients and families on health management and self-care.
• Collaborate with multidisciplinary team to ensure outcomes.
