Job summary

Well Care Health is seeking a compassionate Home Health Registered Nurse to provide in-home patient care and support. You will assess health conditions, develop care plans, educate families, and coordinate with a multidisciplinary team to achieve optimal outcomes.

The role requires an RN license in NC or SC, CPR certification, and at least one year of RN experience with preference for home health background. Flexible scheduling and strong teamwork are offered.

Qualifications

• Graduate of an accredited or approved school of nursing (AD, Diploma, or BSN).
• Current license to practice as a registered nurse in NC/SC; CPR certification.
• Minimum 1 year RN experience; 2+ years total clinical experience; home health experience preferred.

Responsibilities

• Assess and monitor patients in their home environment.
• Develop and implement individualized care plans.
• Educate patients and families on health management and self-care.
