Job summary

Well Care Health is seeking a Home Health Registered Nurse to provide comprehensive patient care in the home setting across life stages, from adolescents to geriatrics. You will assess, plan, implement, and evaluate care using the nursing process, coordinate with a multidisciplinary team, and ensure adherence to agency policies while promoting patient satisfaction.

The role requires NC/SC RN licensure, CPR certification, and a track record of at least 1 year of RN experience with home health

Qualifications

• Graduates from an accredited nursing program (AD, Diploma or BSN).
• Current license to practice as a registered nurse in NC/SC; CPR certification required.

Responsibilities

• Assess, plan, implement and evaluate care using the nursing process for home-based patients.
• Coordinate care with a multidisciplinary team to manage caseload.
• Communicate policy-adherent information to patients and families.
• Contribute to program effectiveness and agency mission.
• Maintain schedules to optimize agency performance.
