Well Care Health in Raleigh, NC is seeking a compassionate Home Health Registered Nurse to assess patients at home, develop care plans, and educate families for better health outcomes.

The role requires an active NC (or SC) nursing license, CPR, and 1 year RN experience with 2+ years total clinical work. You will collaborate with a multidisciplinary team and support growth within a patient‑centered home health program.

Qualifications

• Education: Graduate of an accredited or approved school of nursing (AD, Diploma, or BSN).
• Licensure: Current RN license in NC (or SC) with CPR certification.
• Experience: 1 year RN, total 2+ years clinical; home health preferred.

Responsibilities

• Assess and monitor patients in their home environment.
• Develop and implement individualized care plans.
• Educate patients and families on health management and self-care.
• Collaborate with a multidisciplinary team to ensure optimal outcomes.
• Communicate in accordance with agency policies and guidelines.
