Job summary

Well Care Health is seeking a compassionate Home Health Registered Nurse to provide patient-centered care in patients' homes across the Carolinas. The role emphasizes applying the nursing process—assessment, planning, implementation, and evaluation—to deliver individualized care for adults, geriatrics, and other life stages in home settings.

The position requires an active NC/SC nursing license, CPR, and at least one year of RN experience with a total of two or more years in clinical settings;

Qualifications

• Graduate of an accredited school of nursing (AD, Diploma, or BSN).
• Current nursing license in NC/SC; CPR certification.
• One year RN experience and at least 2 total years of clinical experience; home health experience preferred.

Responsibilities

• Assess and monitor patients in their home.
• Develop and implement individualized care plans.
• Educate patients and families on health management and self-care.
• Collaborate with a multidisciplinary team to achieve optimal outcomes.
• Maintain and promote patient satisfaction and quality of care.
