Job summary

Aveanna Healthcare in Georgia seeks a Registered Nurse (Home Health) for salaried full-time work in N Dekalb County. You will deliver one-to-one nursing care at patients' homes, perform in-home assessments, and develop patient-centered care plans.

Responsibilities include wound care, infusion and medication management, patient/caregiver education, and coordinating with the interdisciplinary team to ensure quality outcomes and prevent hospitalizations. Driving is required.

Qualifications

• Active RN license in the state of practice.
• At least 1 year of clinical nursing experience.
• Current driver’s license and reliable transportation.
• CPR certification (or ability to obtain).

Responsibilities

• Deliver one‑on‑one in-home nursing care to promote recovery, safety and independence.
• Conduct comprehensive in‑home assessments and translate findings into care plans.
• Provide skilled nursing interventions including wound care, infusions, catheter care, medication management and post‑operative care.
• Educate patients and caregivers to manage symptoms, medications and daily care at home.
• Monitor progress, identify changes, and communicate with physicians and the care team to adjust plans.
• Coordinate care across providers to ensure continuity and high‑quality outcomes.
• Complete accurate documentation reflecting the clinical picture and outcomes.
