Aveanna Healthcare is seeking a Registered Nurse (Home Health) to provide one-on-one in-home nursing care in Cobb County, GA, with a Monday–Friday daytime schedule and a 7‑day on-call rotation. You will assess, treat, educate, and coordinate care to promote recovery and independence for patients, while ensuring high-quality outcomes.

Qualifications

• Active RN license in the state of practice.
• Minimum of 1 year of clinical nursing experience.
• Current driver's license, reliable transportation, and auto insurance.
• CPR certification (or ability to obtain).

Responsabilités

• Serve as the primary clinical partner for patients and families, delivering in-home one-on-one nursing care that promotes recovery, safety, and independence.
• Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, and translate findings into patient-centered care plans.
• Provide skilled nursing interventions—including wound care, infusions, catheter care, medication management, post-operative care, and chronic disease management—tailored to each patient’s goals.
• Educate and empower patients and caregivers through clear instruction, coaching, and support, helping them manage symptoms, medications, and daily care at home.
• Monitor patient progress closely, identify changes in condition early, and communicate with physicians and the care team to adjust plans and prevent hospitalizations.
• Coordinate care across providers, ensuring continuity and high-quality outcomes throughout the home health journey.
• Complete timely documentation reflecting the clinical picture and supporting quality and compliance.
