Job summary

Aveanna Healthcare in Delaware seeks an experienced Registered Nurse to join our Home Health team for full-time in-home visits in Sussex County. You will deliver personalized care, perform in-home assessments, and manage treatments to promote recovery, safety, and independence.

Responsibilities include wound care, IVs, catheter care, medication management, and patient education, with collaboration across an interdisciplinary team.

Qualifications

• Active RN license in the state of practice.
• Minimum 1 year of clinical nursing experience.
• 1 year of Home Health nursing experience.
• Must have Oasis documentation experience; HCHB experience preferred.

Responsibilities

• Deliver one-on-one in-home nursing care.
• Conduct comprehensive in-home assessments and develop care plans.
• Provide skilled nursing interventions including wound care, infusions, catheter care, and medications.
• Educate patients and caregivers on self-care and symptom management.
• Monitor progress and communicate with physicians to adjust plans.
• Coordinate care across providers to ensure continuity and quality.
• Complete timely, accurate documentation reflecting the clinical picture.
