Aveanna Healthcare is seeking a Registered Nurse (Home Health) for Decatur and Priceville areas. This full-time role provides one-on-one in-home nursing care, comprehensive assessments, and skilled interventions to support patient recovery and safety.

A strong emphasis is placed on education, coordination with the care team, and documentation for quality outcomes. We value compassionate, patient-centered care delivered in the home, with opportunities for professional growth and ongoing CEUs.

Qualifications

• Holds an active Registered Nurse (RN) license in the state of practice.
• At least 1 year of clinical nursing experience.
• CPR certification (or ability to obtain).
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver in-home one-on-one nursing care to promote recovery, safety and independence.
• Conduct comprehensive in-home assessments and translate findings into patient-centered care plans.
• Provide skilled nursing interventions including wound care, infusions, catheter care, medication management, post-operative and chronic disease management.
• Educate and empower patients and caregivers to manage symptoms and daily care at home.
• Monitor progress and communicate changes to physicians and care team to prevent hospitalizations.
• Coordinate care across providers to ensure continuity and high-quality outcomes.
• Complete timely documentation reflecting the clinical picture for quality and compliance.
