Job summary

BrightStar Care is seeking a compassionate Home Health Registered Nurse (RN) to provide one-on-one skilled care in patients' homes in Quinlan, TX and surrounding areas.

You will perform SOC/evaluation, wound and catheter care, medication management, and care coordination with physicians and the care team, while promoting safety and independence.

Qualifications

• Current Texas RN license in good standing.
• Current CPR/BLS certification.
• Minimum 1 year of nursing experience.
• Valid driver's license and reliable transportation.
• Excellent communication and organizational skills.

Responsibilities

• Complete Start of Care (SOC), Recertification, Resumption of Care, Evaluation, and Discharge visits.
• Perform comprehensive nursing assessments and develop individualized plans of care.
• Monitor patient conditions and communicate changes in health status to the care team.
• Provide wound care and catheter care, administer medications, and monitor for adverse reactions.
• Coordinate care with physicians, therapists, patients, caregivers, and other healthcare professionals.
• Educate patients and caregivers on disease management and home safety.
• Maintain accurate electronic documentation and HIPAA compliance.
