Job summary

Life Care Home Health is seeking a registered nurse to administer nursing care for clients in their residences and coordinate care with the interdisciplinary team, clients, families, and the referring agency.

You will perform assessments, develop and revise the Plan of Care, communicate with the team, ensure documentation and infection control, supervise other nursing personnel, and participate in on-call responsibilities.

Qualifications

• One (1) year experience as an RN in a clinical care setting.
• Preferred Home health experience.
• Completion of an accredited Registered Nursing Program.
• Preferred, BSN.
• Current driver’s license in good standing and reliable transportation.
• Current State RN license and CPR certification.

Responsibilities

• Administer nursing care for clients in their residence.
• Coordinate care with interdisciplinary team, clients, families, and referring agency.
• Perform initial and ongoing assessments (including OASIS).
• Develop, initiate, and revise the Plan of Care to ensure quality and continuity of care.
• Provide effective communication to clients, families, and health care professionals; participate in case conferences.
• Ensure infection control and universal precautions in patient care activities.
• Coordinate services and inform physicians of changes in client condition.
• Monitor cases for third-party payer requirements.
• Complete documentation timely to meet policy.
• Supervise and provide clinical direction to LVN/LPN/CNA staff.
• Perform on-call duties as assigned.
• Maintain ongoing professional development and licensure.
