Job summary

Pinnacle Home Care in Florida is seeking a Registered Nurse (RN) to join our award-winning home health team on a PRN basis. You will design and implement patient-specific care plans, perform comprehensive assessments, and coordinate care with physicians, discharge planners, and family caregivers.

We value experienced RNs with valid state license and BLS, who can travel to patient homes across Florida, maintain accurate documentation, and contribute to our Quality Assurance program while

Qualifications

• Must hold a current RN license (state-specific).
• Valid BLS certification.
• Minimum one year of experience in home health or healthcare facility.
• Familiarity with Medicare and Medicaid guidelines.

Responsibilities

• Design and implement patient-specific care plans focused on recovery.
• Provide nursing care aligned with the care plan, including assessments.
• Monitor patient conditions and coordinate health promotion and direct care.
• Maintain detailed clinical records and required documentation (OASIS).
• Communicate with clinical managers and colleagues to ensure quality care.
• Provide training to patients and families/caregivers.
• Serve as a liaison between patients and community health resources.
• Collaborate with physicians and discharge planners on health education.
• Confirm daily visit schedules for seamless coordination.
