Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, is hiring PRN Registered Nurses to support patients in Boca Raton and surrounding areas. You will design and implement patient-specific care plans, perform comprehensive assessments, and coordinate with physicians and caregivers to promote recovery at home.

Qualifications

• Must hold an active RN license (state-specific).
• BLS certification required.
• Minimum 1 year of experience in home health or healthcare facility.
• Familiar with Medicare/Medicaid guidelines.

Responsabilités

• Design and implement patient-specific care plans focused on recovery.
• Provide high-quality nursing care in line with care plans and perform assessments.
• Monitor patients and coordinate health promotion, prevention, and direct care.
• Maintain detailed clinical records, including OASIS documentation.
• Communicate with clinical managers and colleagues to ensure quality care.
• Provide training and support to patients and families/caregivers.
• Serve as a liaison between patients and community health resources.
• Collaborate with physicians and discharge planners on health education.
