PurposeCare is seeking a Home Care Registered Nurse Case Manager to coordinate patient-centered home health services in the Indiana region. You will assess, plan, and manage care for clients in their homes, collaborating with clinicians and caregivers to ensure quality outcomes.

Qualifications
- Current license with the state Board of Nursing.
- 1 year of nursing experience is required - home care experience preferred.
- Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
- Current Driver's License, vehicle insurance, and/or access to dependable transportation
- Ability to successfully complete a background check
