Responsibilities

• Coordinates direct and indirect patient care using the nursing process.
• Performs skilled nursing assessments and care in home settings.
• Completes OASIS/485 changes within 24 hours of receiving them.
• Documents accurately and timely, including admission and discharge notes.
• Collaborates with the health care team to plan and coordinate services.
• Supervises Home Health Aide activities and ensures proper care plans.
