Qualifications

• RN licensed in Idaho
• Valid Driver’s License and Auto Insurance
• Home Health and/or Hospice experience preferred

Responsibilities

• Performs initial patient assessment and develops a care plan with patient/family
• Maintains ongoing evaluation and updates to primary physician
• Administers medications and treatments as prescribed, coordinates care with team
• Participates in on-call duties and ensures proper documentation
