Qualifications

• RN licensed in Idaho and current licensure to practice home health nursing.
• Driver's license required to travel to patient locations as needed.
• Home Health and Hospice experience is preferred.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs and provides a complete comprehensive assessment and history.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and revises as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and includes patient and family in planning.
• Initiates preventive and rehabilitative nursing procedures and administers prescribed medications/treatments.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions appropriate to assessment and plan of care.
• Identifies discharge planning needs and coordinates care prior to discharge.
• Acts as Case Manager when assigned by Clinical Supervisor or designee to coordinate patient care.
• Adheres to documentation and care procedures and standards of conduct.

Education

RN licensed in Idaho

Valid Driver's License
