Qualifications

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs and develops a care plan.
• Regularly re-evaluates patient nursing needs and initiates revisions as status changes.
• Administers medications and treatments as prescribed by the physician and counsels patient and family.
• Prepares clinical notes and communicates with physicians and community health professionals as needed.
• Participates in on-call duties and coordinates equipment and services for patient care.
• Identifies discharge planning needs and coordinates with the care team prior to discharge.
