Job summary

Horizon Home Health & Hospice in Burley, ID is seeking a full-time Registered Nurse (RN) to provide compassionate in-home patient care. You will perform assessments, develop and revise care plans, administer medications, and coordinate with physicians and families.

Qualifications

• RN licensed in the State of Idaho is required.
• Valid Driver's License is required.
• Home Health and Hospice experience preferred.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs and develops a comprehensive plan of care.
• Regularly re-evaluates patient nursing needs and revises the plan as status changes.
• Administers medications and treatments as prescribed by the physician.
• Coordinates patient care with the primary physician and other health care professionals.
• Acts as Case Manager when assigned and coordinates care for the caseload.
• Maintains documentation and adheres to policies and professional conduct.
