Zusammenfassung

Interim HealthCare of Central Montana is seeking a Home Health Registered Nurse to provide compassionate, patient-centered care in the home setting. You’ll manage care plans, educate patients, and collaborate with a team of LPNs, CNAs and aides to support recovery and independence.

We offer flexible assignments, 1:1 patient ratios where possible, and opportunities for ongoing training and CEUs. A Montana RN license and CPR certification are required, with a focus on quality outcomes and family

Qualifikationen

• RN license in Montana is required.
• CPR certification is required.
• Knowledge of state and federal home health regulations is required.
• Clinical proficiency in nursing and strong communication skills are essential.
• Qualities include kindness, empathy, compassion, honesty, integrity, dignity, respect, patience and understanding.

Aufgaben

• Provide home-based care to patients after discharge from hospital, rehab or facility.
• Oversee LPNs, CNAs and aides providing patient care.
• Examine patients, perform assessments, document progress and report changes to physician.
• Educate patients on plan of care, medications, therapy, diet and exercise.
• Coach family members on plan of care, medications and home safety.
• Coordinate discharge from services and ensure goals are met.
