Job summary
Interim HealthCare of Central Montana is seeking an experienced Home Health Licensed Practical Nurse (LPN) to join our team. You will provide home-based care under RN supervision, collaborate with RNs and CNAs, and help patients manage illnesses and chronic conditions at home.
You’ll take vitals, perform assessments, and educate families on care plans and safety. We offer flexible schedules, autonomy, and full-time benefits including PTO, vision, and dental. Montana LPN license required.
Qualifications
- Graduate of an accredited practical nursing program.
- Active LPN license in Montana.
- CPR certification.
- Knowledge of state and federal home health regulations.
- 1 year of LPN experience in home health.
- Strong communication skills and compassion.
Responsibilities
- Provide home-based care to patients under supervision of an RN.
- Collaborate with RNs, CNAs and aides to deliver quality patient care.
- Take patient vitals, perform assessments, document progress and report changes to the RN supervisor.
- Assist RN with plan of care and teach patient self-care techniques.
- Educate family on plan of care, medications and home safety.
- Coordinate with interdisciplinary team for discharge planning.
