Job summary

PurposeCare is a leading home health and home care provider in the Midwest, serving 3,000+ clients monthly. We are seeking caring HHAs/CNAs to deliver personal care in clients’ homes with one-on-one attention. Expect flexible scheduling and opportunities to grow with a compassionate, fast-growing team.

You’ll be hands-on, supporting daily living needs and ensuring safe, comfortable aging in place. Licensed or willing to become licensed candidates are encouraged to apply.

Qualifications

• Active HHA or CNA certification or willingness to obtain certification.
• Ability to follow written care instructions and document services.
• Reliable transportation and ability to travel to client homes.
• Willingness to complete TB testing, background screening, and onboarding requirements.
• Compassion, patience, and professionalism with vulnerable populations.
• Ability to lift up to 25 pounds.
• Willingness to complete competency assessments.

Responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting.
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan.
• Prepare meals and assist with feeding as needed.
• Support medication self-administration through reminders (no independent med administration).
• Monitor and report patient condition and changes to the supervising nurse.
• Take and record basic vital signs when trained and assigned.
• Maintain a clean, safe, and orderly client environment.
• Assist with linens and light client laundry as appropriate.
• Use safety equipment properly (rails, supports, stools, etc.).
• Document care provided according to company and regulatory standards.
• Travel reliably to patient homes and assigned visits across your service area.
• Complete required in-service training, including annual dementia education.
