Job summary

One Home Health LLC is seeking a dedicated Registered Nurse (RN) to provide high-quality home health care in Niles, IL and surrounding areas. The role includes patient assessment, wound and IV care, and education for patients and caregivers within a compliant home health setting.

The RN will perform initial visits, coordinate with multidisciplinary teams, supervise Home Health Aide staff as needed, and ensure timely documentation consistent with policy and Medicare regulations.

Qualifications

• Observation, assessment, and documentation of patient symptoms and progress.
• Perform initial home care visit and re-evaluate patient needs and progress when needed.
• Demonstrate efficient teamwork with the staff.
• Demonstrate competence in documenting Oasis.
• Educate patients and caregivers on disease processes, plan of care, and treatment plans.

Responsibilities

• Provide wound care and assessment.
• Provide IV line care and maintenance.
• Coordinate other disciplines as applicable to assist patient.
• Follow Medicare regulations and notify physician of changes in patient condition.
• Complete paperwork timely and efficiently.
• Supervise HHA field staff as indicated.
