Job summary

SSM Health at Home Mount Vernon is seeking a dedicated Certified Nursing Assistant to provide essential nursing care in a home health and hospice setting. You will receive assignments from the RN and perform delegated tasks to support patient comfort and safety.

This full-time, 32-hour weekly schedule requires an Illinois CNA certification and a valid Illinois driver's license. BLS HCP is preferred, and duties include basic clinical tasks, documentation, and patient education.

Qualifications

• Illinois CNA certification is required.
• Completion of an approved nurse aide training program.
• No prior work experience required.

Responsibilities

• Receives patient care assignment from the RN in charge and initiates delegated tasks.
• Collects and documents data for admissions and reassessments for the patient population.
• Supports safety and quality by relaying information via charting and communication.
• Notifies the licensed nurse of changes in patient condition or needs.
• Assists with activities of daily living and routine care, including baths, mobility, hygiene, and basic treatments.
• Prepares patients for discharge and accompanies them as needed.
• Stocks supplies and maintains equipment while managing time efficiently.
