Job summary

Fortcom Health Services is seeking a compassionate Full-Time Field/Office Registered Nurse to join our Adult Family/Foster Care team. You will conduct admissions assessments and develop care plans for medically eligible adults, supporting caregivers and coordinating with physicians.

You will also perform periodic in-home visits, complete required clinical documentation, and ensure paperwork is accurate before services begin.

Qualifications

• Active MA RN license in good standing is required.
• Degree in nursing with BSN preferred; field/home health experience is a plus.

Responsibilities

• Conduct admissions assessments and complete required nursing documentation.
• Perform in-home supervisory visits every other month as per requirements.
• Develop and maintain Plans of Care with input from caregivers and Program Director.
• Coordinate medical clearances and summaries with physicians.
• Provide caregiver orientation and ongoing training.
• Enter referral and member data into AFC systems as needed.
• Ensure compliance with care standards and participate in audits.
